    Use Google to find one ... gazillion of them.
    Some use AJAX, PHP, JQuery ... some have captcha, validation, CSS/HTML emails ...

    Forms are insanely easy to make, a simple form without security can be made in about 3 - 4 minutes, one with average security about 10 - 15 minutes. If you want a really secure form, you're looking at around an hour tops.
