You don't. You have to set the keys up in the server configuration. Most shared hosting panels have a method to do this automatically.

This is one of those questions that if you don't know what it is or don't know how to do it, you probably don't need it and should not be doing it.

SSL certificates are domain specific. They have nothing to do with php or specific web pages. You need a dedicated IP address for them to work, as requests made to the IP address that the domain corresponds to must resolve directly to only one domain name (if you have shared hosting, you must ask your web host to assign a dedicated IP address to your domain.) Finally, you must have access to the web server to install the certificate, which generally would require your web host to install it for you (I believe some hosting control panels will allow you to install a certificate yourself.)

Do you have a specific question about what you should do that was not answered by researching this yourself and checking with your web host for information they have that is specific to their setup?
If you are learning PHP, developing PHP code, or debugging PHP code, do yourself a favor and check your web server log for errors and/or turn on full PHP error reporting in php.ini or in a .htaccess file to get PHP to help you.
