    Radio button in a prompt?

    I am creating a page for reserving flights (its homework.)
    In the assignment my professor asks for a prompt notifying the customer that once they hit submit they are agreeing to book the flight. He says that this should be followed by a radio button with the options I agree and I disagree. I can't ask him what he means by this and I cannot find a code for a prompt containing a radio box. Any advice?

    Take a look at this.

    click 'add new small add'. This resembles prompt,by its look, and partly by its 'modal' mode, it can have any control on it.

    js generated, but you could do some div wih initial display:none
