    Question Survey Creator

    Not sure if this is the right place to post this, but can someone give me some links to some recommended free survey creator sites. I am wanting to create a customer satisfaction survey for a site, but I am on a $0 budget, so I need a free one.

    I searched Google, but when looking for a form builder, I found a better one through a forum, than any of the ones that they recommended. So I'm hoping the same can happen again here.

    Give us some specs ....

    1) How many questions?
    2) Do they need to be logged-in to do the survey?
    3) Can they do the survey more than once, by PC, or by user?
    4) Can they do the survey with cookies disabled?
    5) What happens with the results?
    6) Can anyone see the results, or only the site owner?
    7) Do they have to be notified by email after the survey?
    8) Is the survey 1-5, 1-10, or by words (seldom, sometimes, always)?
    9) Is this a one-time survey, or will it change each week, month, etc?

    Hard to steer you in the right direction with so little information.

    1) approx 5-10 questions. Not quite sure yet
    2) They do not need to be logged in. I just want to send them the link once their order is finished.
    3) Preferably only once, as the survey is asking what they thought of our service.
    4) Could you tell me what difference would this make? I haven't had any experience with cookies
    5,6,7) I would just like for the results to be emailed to me, and maybe have a webmasters panel through the site or something that I can check too.

    9) There will be a new one 4 times a year.

    This survey is because I am a leader at a kidz camp, and we have a media team that I head for the camp. We spend the whole week filming, taking photos, making movies etc and at the end of camp we make a camp DVD. These DVD's are then available for sale through our website www.kctv.info where you can email us to order one. We are just wanting to create a survey for the parents to fill out on questions based on how they thought our service was.

    Any help would be great.

    Only a logged-in user would be controllable ... one survey based on their login.
    But you don't require them to be logged-in.

    If they don't need to be logged-in to do the survey, anyone could do it, at any time.
    You would not know who did it, except by IP (which can change).

    So, cookies would only allow 1 PC to do it ... unless they erased their cookies.
    Some people disable cookies, so they could do the survey several time if they wanted.

    You didn't ask this in the PHP section, but you would use PHP to do the survey.
    The site is actually displayed by an HTML frame via dpdesignz.net
    dpdesignz.net is hosted by GoDaddy, so you can use PHP.

    Do you know anything about PHP?

    Do the survey questions appear all one on page, or displayed one by one?

    I don't know how to say this without sounding like I'm being too critical, because
    it's hard to express emotion using a forum ... but, the site looks very poorly done,
    and not very appealing. I certainly don't mean to offend you, because you probably
    worked hard on it, but I have to be honest ... we're here to help people out.

    On IE8, the layout is broken (looks way different than FF), the text (contrast) is
    too hard to read, and the font size and colors are impossible to see.

    It should be done using a mix of XHTML/CSS and PHP ... to give the site owner
    capabilities of doing a survey, editing content. PHP is pretty much a requirement
    for sites these days.

    If you are willing to use some PHP scripting, I can help you with the survey portion.

    I am actually wanting to know if there is a web-based builder I can use. ie i use jotform to do my online forms.

    As to the iFrame. dpDesignz is my own personal site. The actual site in question, KCTV, is also hosted by GoDaddy, except I am just sorting some things out with my team about hosting, so I'm just doing a masked host through my site for now so my team can preview the site.

    I do know a bit about php, but I'm wanting to leave that as a last resort as I said I would prefer a web based builder.

    I don't mind if they appear on one, or heaps.

    No offence taken about the site.
    We have actually stopped working on that design because we are completely redesigning it on a localhost. Mainly for the fact that our current design is so broken across the various browsers. That version is kind of obsolete, and is just there for a kind of splash page for now. It's not publicly launched yet.

    Thanks for the offer of the help. Would you charge? Unfortunately as this is a non-profit group project, I have a $0 budget.

    This one will be "on me" ... no charge.

    You can work the survey from a plain-text file and use PHP to process it.

    Create a text file (using Notepad) called, "survey.db" ...
    You can use .db, or any other extension ... I just like to not use .txt, because
    it would be easy for someone to figure-out the extension, and view the file.

    In the file "survey.db", create one line for each question.
    The fields are separated by pipes |

    DATE | ID | QUESTION

    Example:

    2010-08-30|10|Was the video download process easy to understand?
    2010-08-30|11|How do you rate the quality of the video?
    .
    .
    .
    2010-10-13|24|How do you rate our email response?
    2010-10-13|25|Was our staff courteous?
    .
    .
    .



    The file contains all questions for as many surveys as you want.
    You just keep adding questions / editing them / deleting them.
    Upload the new "survey.db" (using FTP) every time you wish to modify it.

    The DATE field controls when the questions will appear (the date of the survey).
    The ID field identifies the question, each line has a unique ID (you determine it).
    When the results are emailed to you, the PHP script will use the ID to show you
    the question(s) they answered, and the rating they gave it.

    In my example ...
    On Aug 30, 2010 ... only those questions will appear, until the next date occurs,
    then those questions will appear, in the order in which they are listed. You can
    add survey questions far in advanced.

    Once you have the file created, show me the file.

    I will then give you a simple PHP script that you can insert into any page you wish.
    The PHP script will have a spot for you to enter your email address, and define where
    the user will be sent to after they submit the survey.

    1) Can I not put a date in, and it will always be there?
    2) Is it possible to have an end date?

    How about reversing the date ...

    It will display everything "equal to or greater" than today's date?

    Example it would only display questions from 2010-08-27 to the future.
    Anything older than today will not show.

    If you don't want dates, then every question in the file would appear.
    That would be OK ... but I thought maybe it would be nice to add or drop them
    without erasing them totally.

    Another idea ... you have a category number.
    Only certain categories display certain questions.
    You define the category when you display the survey.
    So there would be no dates, but you control which questions appear.

    How about that ... example, category 2 only displays id-127 and id-25 ...

    1|10|Was the video download process easy to understand?
    1|11|How do you rate the quality of the video?
    2|127|How do you rate our email response?
    2|25|Was our staff courteous?

    the text file for me ... and how you want it to work? by date, by category,
    or display all questions in the file?

    Give me a "survey.db" file to work with. The PHP part is easy, I'll do that.

    and a specification on how it should display questions, by date, category, everything, etc.?
